How to make Tracking

Hi everyone,
I was trying to use tracking module, for that i have made markbook for one class and 4 result in that, and when i am going to track the record for that class it is showing me something like this

what should i do so that i can track records and what does 4 data points means if you can explain how it i should do it would be great
