Import from file > Staff (Staff Management)

Hi
I have tried to add staff using the DataStructure-staff template, however only the first staff member gets added, then an error for Field (-1) occurs for each row, with Message - Failed to insert or update database record.
I don't understand what I have done wrong...
Help much appreciated!
